A Structured Approach in Fetal Ultrasound

Conotruncal anomalies are disorders involving the outflow tract and great arteries.

대표적으로:

• Tetralogy of Fallot (TOF)
• Double Outlet Right Ventricle (DORV)
• Transposition of the Great Arteries (TGA)
• Truncus arteriosus
• Pulmonary atresia
• Interrupted aortic arch

이들은 모두

👉 Outflow tract와 great vessel alignment 이상이 핵심입니다.

🔥 Step 1: 정상 구조를 이해하자

정상 심장에서는:

• LV → Aorta
• RV → Pulmonary artery
• Great vessels는 서로 교차(crossing pattern)

이 “crossing”이 깨지면 conotruncal anomaly를 의심합니다.

🔎 Step 2: 구조적 분류

1️⃣ Pulmonary Obstruction Dominant

• TOF
• Severe pulmonary stenosis
• Pulmonary atresia

👉 PA가 작아짐

👉 Ao dominance

👉 3VT에서 asymmetric V

2️⃣ Abnormal Origin

• DORV
• TGA

👉 Great vessels 평행

👉 LV 연결 확인 필수

3️⃣ Single Outflow Tract

• Truncus arteriosus

👉 3VT에서 2-vessel appearance

👉 두 대혈관 분리 안 됨

4️⃣ Arch Abnormality

• Right aortic arch
• Double aortic arch
• Interrupted aortic arch

👉 3VT에서 U-shape 또는 ring

👉 Trachea 위치 중요

When 3VT is abnormal:

1️⃣ PA 크기 확인

2️⃣ V-shape 유지 여부 확인

3️⃣ Great vessel crossing 있는가?

4️⃣ LVOT 직접 연결 확인

5️⃣ Ductus flow 방향 확인
