During your baby’s anatomy scan, the sonographer carefully examines several structures inside the fetal brain.
One of them has a long name:
Cavum Septi Pellucidi — or CSP.
Most parents have never heard of it before pregnancy.
So what exactly is the CSP, and why do we look for it during a fetal ultrasound?
The CSP is a small fluid-filled space located near the center of the developing fetal brain.
Seeing it at the appropriate stage of pregnancy is one of several landmarks that help healthcare providers evaluate normal midline brain development.
What Is the CSP?
The cavum septi pellucidi is a small space between two thin membranes called the leaflets of the septum pellucidum.
On ultrasound, it usually appears as a small dark—or fluid-filled—space near the center of the brain.
It is located beneath the corpus callosum and above the fornix.
Because of this location, the CSP is an important landmark when evaluating fetal brain anatomy.
What Does the CSP Look Like on Ultrasound?
In the correct ultrasound plane, the CSP typically appears as a small fluid-filled space with thin echogenic borders.
Parents sometimes describe it as:
“a little black box”
or
“a small dark space in the middle of the brain.”
That dark appearance is expected because fluid appears dark on ultrasound.
So seeing a dark CSP is not a hole or missing part of the baby’s brain.
It represents a normal developmental space.
Why Do Sonographers Check the CSP?
The CSP is part of the standard evaluation of fetal intracranial anatomy during the mid-trimester ultrasound.
Its presence helps confirm that certain midline brain structures have developed in the expected relationship.
But the CSP should never be interpreted by itself.
The sonographer also evaluates structures such as:
- lateral ventricles
- thalami
- cerebellum
- cisterna magna
- corpus callosum when indicated
- other midline structures
Think of the CSP as one landmark within a much larger brain examination.
When Can the CSP Be Seen?
The CSP becomes visible during fetal development and is normally seen during much of the second and third trimesters.
This is why it is routinely assessed during the 18–22 week anatomy scan.
Visibility can vary depending on gestational age, fetal position, imaging plane, and image quality.
So:
“I couldn’t see it on one image” does not automatically mean the CSP is absent.
Obtaining the correct plane matters.
Why Does the Imaging Plane Matter?
This is especially important with the CSP.
Fetal brain structures can look very different depending on the angle of the ultrasound beam.
If the image is slightly too high, too low, or oblique, the CSP may not be clearly demonstrated.
Before concluding that a CSP is truly absent, the examiner may:
adjust the imaging plane → change the probe angle → wait for fetal movement → obtain additional brain views.
Sometimes a repeat ultrasound is needed simply because the baby was not in an ideal position.
What If the CSP Is Not Seen?
Not seeing the CSP on an ultrasound image does not automatically mean there is a brain abnormality.
The first question is whether it is truly absent or simply difficult to visualize.
If the CSP remains nonvisualized after careful evaluation, however, the healthcare team may take a closer look at the fetal brain.
This is because absent or abnormal visualization of the CSP can sometimes be associated with other developmental abnormalities.
The next step may include a more detailed neurosonographic examination.
What Conditions Can Be Associated With an Absent CSP?
Persistent nonvisualization of the CSP can occur with several different fetal brain conditions.
Examples may include abnormalities involving:
- the corpus callosum
- holoprosencephaly
- septo-optic dysplasia
- severe hydrocephalus
- other midline developmental abnormalities
But this list should not be used to diagnose a baby from a single ultrasound image.
The significance depends on what the rest of the fetal brain looks like.
Does an Absent CSP Always Mean Something Is Wrong?
No.
There are situations in which the CSP may be difficult to visualize because of technical factors.
And even when the septal leaflets are genuinely abnormal or absent, the clinical significance depends heavily on whether other brain abnormalities are present.
This is why doctors do not diagnose a condition simply by saying:
“I can’t see the CSP.”
Instead, they evaluate the entire fetal brain.
What Happens If the CSP Is Difficult to See?
Your healthcare provider may recommend:
repeat ultrasound
or
detailed fetal neurosonography.
In selected cases, additional imaging such as fetal MRI may be considered if it could provide useful information about fetal brain anatomy.
Again, fetal MRI is not automatically needed simply because the CSP was difficult to see on one scan.
Is the CSP the Same as the Corpus Callosum?
No.
They are different structures, although they are anatomically related.
The corpus callosum is a large bundle of nerve fibers connecting the two cerebral hemispheres.
The CSP is a fluid-filled space between the septal leaflets beneath the corpus callosum.
Because of their anatomical relationship, an abnormal CSP finding may sometimes prompt closer evaluation of the corpus callosum.

Does the CSP Stay Forever?
Not necessarily.
The CSP is a developmental structure.
It commonly becomes smaller and closes later in infancy, although persistence can occur.
Therefore, the CSP seen during pregnancy is not expected to remain as the same open space throughout a person’s entire life.
What Should I Ask If My Baby’s CSP Wasn’t Seen?
Useful questions include:
- Was the CSP truly absent or just difficult to visualize?
- Was the baby’s position limiting the examination?
- Does the rest of the fetal brain look normal?
- Are the lateral ventricles normal?
- Does the corpus callosum appear normal?
- Do I need a repeat ultrasound?
- Would detailed neurosonography be helpful?
- Is any additional imaging actually necessary?
One of the most useful questions is:
“Is this an isolated finding, or did you see anything else abnormal in the brain?”
That distinction can make a major difference in how the finding is interpreted.
