Fibroadenoma on Ultrasound: What Does It Mean?

You had a breast ultrasound and were told:

“This looks like a fibroadenoma.”

You may immediately wonder:

Is it cancer? Can it become cancer? Why is it BI-RADS 3? Do I need a biopsy?

A fibroadenoma is one of the most common benign solid breast masses, particularly in younger women.

Many fibroadenomas have a characteristic appearance on ultrasound and can often be monitored rather than immediately biopsied.

But as with any breast mass, the entire imaging appearance matters.

What Is a Fibroadenoma?

A fibroadenoma is a benign breast tumor made of glandular and fibrous tissue.

It is commonly found in adolescents and women in their 20s and 30s, although it can occur at other ages.

You may have:

  • One fibroadenoma
  • Several fibroadenomas
  • Fibroadenomas in both breasts

Some are discovered because you can feel them, while others are found incidentally during breast imaging.

What Does a Fibroadenoma Feel Like?

A typical fibroadenoma may feel:

  • Round or oval
  • Smooth
  • Firm or rubbery
  • Well-defined
  • Mobile beneath the skin

This mobility is why fibroadenomas are sometimes informally called “breast mice.”

However, you cannot reliably diagnose a fibroadenoma by touch alone.

A benign fibroadenoma and another solid breast mass can sometimes feel similar.

What Does a Fibroadenoma Look Like on Ultrasound?

A typical fibroadenoma often appears:

  • Oval
  • Well-circumscribed
  • Wider than tall
  • Parallel to the skin
  • Relatively homogeneous internally
  • Hypoechoic compared with surrounding tissue

These features can be reassuring.

But no single ultrasound feature confirms a fibroadenoma.

We evaluate the shape, orientation, margins, internal echoes, posterior features, vascularity, and surrounding tissue together.

What Does “Hypoechoic” Mean?

You may see the word hypoechoic in your ultrasound report.

It simply means that an area appears darker than the surrounding tissue on ultrasound.

Many fibroadenomas are hypoechoic.

But this is important:

Hypoechoic does not mean cancer.

Many benign and malignant breast masses can appear hypoechoic, so the complete ultrasound appearance is much more important than this one word.

Why Is a Fibroadenoma Often BI-RADS 3?

A mass with typical benign-appearing features may sometimes be classified as BI-RADS 3: probably benign.

BI-RADS 3 does not mean “probably cancer.”

It means the finding has a very low likelihood of malignancy — no more than 2% — and short-interval imaging follow-up may be recommended instead of immediate biopsy.

The purpose of follow-up is to confirm that the finding remains stable over time.

How Often Is BI-RADS 3 Followed?

Follow-up schedules vary depending on the imaging finding and clinical situation.

A commonly used approach may include imaging around:

  • 6 months
  • 12 months
  • 24 months

The exact schedule should follow the recommendation in your imaging report.

If the mass remains stable for the appropriate follow-up period, it may eventually be considered benign.

Does Every Fibroadenoma Need a Biopsy?

No.

A mass with classic probably benign features may be monitored with imaging.

A biopsy may be recommended when:

  • Imaging features are not typically benign
  • The mass is growing significantly
  • There is imaging-clinical discordance
  • The patient’s circumstances warrant tissue diagnosis
  • The radiologist assigns a suspicious BI-RADS category

A biopsy recommendation does not automatically mean cancer.

It means tissue is needed to determine exactly what the lesion is.

Can a Fibroadenoma Grow?

Yes.

Fibroadenomas can change in size.

Growth may occur during periods of hormonal change, including:

  • Adolescence
  • Pregnancy
  • Other hormonally active periods

Some fibroadenomas become smaller over time, particularly later in life.

However, noticeable growth of a known mass should be reassessed rather than assumed to be harmless.

If My Fibroadenoma Gets Bigger, Does That Mean Cancer?

Not necessarily.

Benign fibroadenomas can grow.

But when a mass changes significantly, healthcare professionals may reconsider whether its behavior still matches the original diagnosis.

Further imaging or biopsy may sometimes be recommended.

Growth is a reason to reassess — not proof of cancer.

Can a Fibroadenoma Turn Into Breast Cancer?

A typical fibroadenoma does not simply transform into breast cancer.

Fibroadenomas are benign tumors.

Cancer arising within a fibroadenoma is rare.

The important issue is not worrying that every fibroadenoma will “turn into” cancer, but making sure a changing or atypical mass is appropriately evaluated.

What Is a Complex Fibroadenoma?

Some fibroadenomas contain additional microscopic features and may be described pathologically as complex fibroadenomas.

This diagnosis is generally made after tissue examination rather than from ultrasound appearance alone.

Your healthcare provider can explain whether any additional follow-up is appropriate based on the pathology and your overall risk factors.

Fibroadenoma vs Breast Cyst: What’s the Difference?

The main difference is what the lesion contains.

A simple cyst is filled with fluid.

A fibroadenoma is a solid mass.

On ultrasound:

Simple cyst

  • Anechoic
  • Fluid-filled
  • Thin-walled
  • Posterior acoustic enhancement

Fibroadenoma

  • Solid
  • Often hypoechoic
  • Usually oval
  • Often circumscribed
  • Commonly parallel to the skin

Ultrasound is very useful for distinguishing a simple cyst from a solid breast mass.

Fibroadenoma vs Breast Cancer on Ultrasound

Typical fibroadenomas often have reassuring features such as:

  • Oval shape
  • Circumscribed margins
  • Parallel orientation
  • Relatively uniform internal appearance

Features that may deserve closer evaluation include:

  • Irregular shape
  • Non-circumscribed or spiculated margins
  • Non-parallel orientation
  • Architectural distortion
  • Other suspicious associated findings

However, there can be overlap.

That is why ultrasound findings are interpreted as a complete pattern rather than using one feature alone.

What Is a Phyllodes Tumor?

A phyllodes tumor is a fibroepithelial breast tumor that can sometimes resemble a fibroadenoma.

Most phyllodes tumors are benign, but some can behave more aggressively.

A rapidly enlarging fibroepithelial mass may require additional evaluation because imaging alone may not always reliably distinguish a fibroadenoma from a phyllodes tumor.

This is one reason significant growth deserves reassessment.

Can Fibroadenomas Hurt?

Yes, although many are painless.

Some people notice:

  • Local tenderness
  • Discomfort before menstruation
  • Sensitivity when pressing the area
  • Awareness of the lump during hormonal changes

Pain does not reliably distinguish a benign mass from a malignant one.

Do Fibroadenomas Need to Be Removed?

Not always.

Many fibroadenomas can remain in the breast without causing problems.

Removal may be considered when a fibroadenoma is:

  • Large
  • Growing
  • Painful or uncomfortable
  • Causing significant breast asymmetry
  • Difficult to distinguish from another lesion
  • Concerning based on imaging or biopsy
  • Causing significant patient concern

The decision depends on the individual situation.

What If I Have Multiple Fibroadenomas?

Some people develop several fibroadenomas.

Having multiple fibroadenomas does not automatically mean you are at high risk for breast cancer.

Each mass is assessed according to its imaging characteristics and changes over time.

If a new lump develops, it should not automatically be assumed to be “just another fibroadenoma” without appropriate assessment.

When Should a Known Fibroadenoma Be Re-Evaluated?

Consider reassessment if:

  • The lump noticeably grows
  • Its shape or feel changes
  • A new lump appears
  • New skin or nipple changes develop
  • There is persistent focal discomfort
  • You miss a recommended imaging follow-up
  • Your healthcare provider recommends additional evaluation

What Should I Look for in My Ultrasound Report?

Instead of focusing only on the word fibroadenoma, look for the final assessment.

Important information may include:

  • Size
  • Location
  • Imaging characteristics
  • BI-RADS category
  • Comparison with previous imaging
  • Follow-up recommendation

The BI-RADS category and recommendation tell you more about the next step than the descriptive word alone.

The Bottom Line

A fibroadenoma is a common benign solid breast mass.

Typical ultrasound features often include:

oval shape + circumscribed margins + parallel orientation + relatively homogeneous hypoechoic appearance

Some typical masses are classified as BI-RADS 3 and followed with imaging rather than immediately biopsied.
