During your baby’s anatomy scan, the sonographer measures several structures inside the fetal brain.
One of those measurements is taken behind the cerebellum:
the cisterna magna.
Most parents have never heard of this structure before pregnancy.
So when an ultrasound report mentions:
“cisterna magna 7 mm”
or
“cisterna magna appears prominent,”
it can be difficult to know what that actually means.
The good news is that the cisterna magna is a normal fluid-filled space in the posterior part of the fetal brain.
The measurement helps us evaluate the posterior fossa, including the cerebellum and surrounding structures.
What Is the Cisterna Magna?
The cisterna magna is a cerebrospinal fluid-filled space located in the posterior fossa, behind the cerebellum.
On fetal ultrasound, fluid appears dark.
So the cisterna magna normally appears as a dark space behind the cerebellum.
It is not:
a cyst
or
a hole in the baby’s brain.
It is part of normal fetal brain anatomy.
Why Do We Measure It?
The cisterna magna measurement is one part of evaluating the fetal posterior fossa.
The examiner also looks at structures such as:
- cerebellum
- cerebellar hemispheres
- vermis
- fourth ventricle
- surrounding posterior fossa anatomy
The measurement therefore should never be interpreted completely by itself.
A number is useful—but the anatomy around that number matters too.
What Is a Normal Cisterna Magna Measurement?
During the usual mid-trimester fetal anatomy examination, the cisterna magna is generally expected to measure approximately:
2–10 mm.
The measurement is typically obtained in the appropriate axial posterior fossa plane.
A measurement within this range is generally reassuring when the surrounding anatomy also appears normal.
What If It Measures 10 mm?
This is where parents often become anxious.
A measurement near 10 mm sits around the upper end of the commonly used reference range.
But ultrasound measurements are not perfectly exact.
Small differences can occur because of:
- imaging plane
- caliper placement
- fetal position
- fetal movement
- examiner technique
So a measurement of 9.8 mm versus 10.1 mm, for example, should not be interpreted as if one baby is completely normal and the other suddenly has a serious abnormality.
The complete examination matters.
What Does “Enlarged Cisterna Magna” Mean?
When the cisterna magna measures more than 10 mm, it may be described as enlarged.
One term you may encounter is:
mega cisterna magna.
But an enlarged measurement alone does not tell us the entire diagnosis.
The examiner needs to evaluate the rest of the posterior fossa carefully.
Is Mega Cisterna Magna the Same as Dandy-Walker Malformation?
No.
This is an important distinction.
Parents searching “large cisterna magna” online often quickly encounter Dandy-Walker malformation.
But:
large cisterna magna ≠ Dandy-Walker malformation.
Dandy-Walker malformation involves additional abnormalities of posterior fossa anatomy, particularly involving the cerebellar vermis and fourth ventricle.
That is why the ultrasound examination focuses on anatomy—not simply the cisterna magna measurement.
What If the Cisterna Magna Looks Small?
A very small or difficult-to-demonstrate cisterna magna can also prompt closer evaluation.
Again, the examiner does not interpret this space alone.
The cerebellum, vermis, posterior fossa shape, and other intracranial structures need to be assessed together.
Technical factors can also affect how the space appears.
Why Does the Ultrasound Plane Matter?
This is especially important when measuring small fetal brain structures.
If the image is too oblique or obtained in the wrong plane, the cisterna magna may appear larger or smaller than it really is.
Before interpreting an unusual measurement, the examiner should make sure the appropriate posterior fossa plane has been obtained.
This is why repeating a measurement is completely normal during an ultrasound examination.
What Else Do We Look at When the Cisterna Magna Is Large?
If the cisterna magna appears enlarged, the examiner may pay particular attention to:
the cerebellar vermis
fourth ventricle
cerebellar hemispheres
posterior fossa configuration
and
the rest of the fetal brain.
The rest of the fetal anatomy may also be reviewed carefully.
The key question becomes:
“Is this an isolated measurement, or is there another anatomical finding?”
Will I Need Another Ultrasound?
Possibly.
If the measurement is borderline, visualization is limited, or the posterior fossa needs further evaluation, a follow-up examination may be recommended.
Detailed fetal neurosonography may also be useful when an abnormality is suspected.
A repeat scan does not automatically mean the finding is worsening.
Sometimes the goal is simply to obtain better views or confirm the measurement.
Will I Need Fetal MRI?
Not every enlarged cisterna magna requires fetal MRI.
MRI may be considered in selected cases when ultrasound suggests a posterior fossa abnormality or when additional anatomical information could change diagnosis or counseling.
Expert ultrasound remains extremely important.
Does an Enlarged Cisterna Magna Mean My Baby Will Have Developmental Problems?
Not from the measurement alone.
The significance depends heavily on whether the finding is truly isolated and whether the rest of the fetal brain and genetic evaluation, when indicated, are reassuring.
An isolated enlarged cisterna magna can have a very different outlook from enlargement associated with other structural abnormalities.
So prognosis should never be based only on:
“How many millimeters is it?”
What Should I Ask My Doctor?
If your baby’s cisterna magna was described as large, small, or borderline, useful questions include:
- What exactly was the measurement?
- Was it measured in the correct plane?
- Does the cerebellum look normal?
- Does the vermis appear normal?
- Is the fourth ventricle normal?
- Is the rest of the fetal brain reassuring?
- Is this currently an isolated finding?
- Should the measurement be repeated?
- Would detailed neurosonography be helpful?
- Is fetal MRI actually necessary?
One particularly useful question is:
“Is the measurement the only concern, or is there an abnormality in the surrounding posterior fossa anatomy?”
Those are very different situations.
