You had an ultrasound a few weeks ago and your amniotic fluid was normal.
At today’s scan, the number is lower.
Maybe your report changed from:
AFI 14 cm → 8 cm
or your deepest pocket is noticeably smaller than before.
Naturally, the first thought may be:
“Why did my amniotic fluid suddenly drop?”
But a lower measurement does not automatically mean that something has gone wrong.
Amniotic fluid is dynamic.
It changes throughout pregnancy, and ultrasound measurements themselves also have some variability.
What matters is not simply whether today’s number is lower than the last one.
The important questions are:
Is the fluid actually abnormal?
How quickly did it change?
Is the baby growing normally?
Could the membranes be leaking?
Are the placenta and fetal circulation reassuring?
First: Amniotic Fluid Is Not a Fixed Number
Amniotic fluid is constantly being produced and removed.
During the second half of pregnancy, fetal urine is a major contributor to amniotic fluid.
At the same time, the fetus continuously swallows fluid.
So fluid volume reflects a changing balance between:
fetal urine production
fetal swallowing
membrane integrity
and
placental and fetal physiology.
Your AFI is not supposed to remain exactly the same at every ultrasound.
Can AFI Change From One Ultrasound to Another?
Yes.
It is completely possible for AFI or DVP to be different at two examinations.
For example:
AFI 15 cm at one scan
and
AFI 11 cm at the next
does not automatically indicate a problem.
Both measurements may still be within an expected range.
The trend becomes more important when fluid:
approaches or crosses the low-fluid threshold,
especially when the change occurs together with another concerning finding.
Why Can the Measurement Be Different?
Several factors can affect the measured fluid volume.
These include:
- true changes in amniotic fluid
- fetal position
- umbilical cord position
- which fluid pockets are accessible
- measurement technique
- maternal hydration
- gestational age
- examiner variability
The baby may simply be lying differently at the second scan.
A pocket that was easy to measure previously may now contain:
a leg
an arm
or
umbilical cord.
That pocket may no longer qualify for measurement.
Why Doesn’t the Sonographer Measure Through the Cord?
When measuring an amniotic fluid pocket, the sonographer tries to identify an appropriate pocket without fetal parts or umbilical cord crossing the measurement area.
Sometimes a large-looking dark space contains cord.
Color Doppler can help reveal it.
Once the cord is identified, the usable fluid pocket may be smaller than it first appeared.
So two apparently similar ultrasound images can produce different valid measurements.
AFI vs DVP: Why Might the Numbers Seem Different?
There are two commonly used ways to assess amniotic fluid.
AFI — Amniotic Fluid Index
Four vertical fluid pockets are measured in different uterine quadrants and added together.
DVP — Deepest Vertical Pocket
The deepest appropriate single pocket is measured.
Because the methods are different, you should not directly compare an AFI number with a DVP number as though they are the same measurement.
For example:
AFI 8 cm is not equivalent to DVP 8 cm.
They use different thresholds and have different meanings.
When Is Amniotic Fluid Considered Low?
A commonly used definition of oligohydramnios is:
DVP < 2 cm
An:
AFI ≤ 5 cm
has also traditionally been used.
Your healthcare team may prefer one method depending on the pregnancy and clinical setting.
A fluid measurement can therefore decrease substantially while still remaining above the diagnostic threshold for oligohydramnios.
That distinction matters.
What If My AFI Is “Borderline”?
You may hear:
“It’s a little low, but not oligohydramnios.”
Terms such as borderline AFI have been used for measurements near the lower end of the range.
There is no single universally applied borderline cutoff.
So rather than focusing on the word “borderline,” clinicians usually consider:
- gestational age
- fetal growth
- previous fluid measurements
- fetal movements
- maternal conditions
- membrane status
- Doppler findings when indicated
A borderline number with everything else reassuring is different from falling fluid associated with fetal growth restriction.
Could I Just Be Dehydrated?
Maternal hydration can influence measured amniotic fluid to some degree.
Studies have shown that increased maternal hydration can sometimes modestly increase measured fluid volume.
However:
A significant fall in amniotic fluid should not automatically be blamed on dehydration.
If the fluid is truly low, clinicians still need to consider other explanations.
Drinking water is sensible during pregnancy.
But it is not a substitute for evaluating significant oligohydramnios.
Could My Water Be Leaking?
Yes.
A decrease in amniotic fluid can occur if the membranes have ruptured.
Sometimes membrane rupture is obvious:
a sudden gush of fluid.
Other times leakage can be more subtle or intermittent.
If you notice persistent watery vaginal leakage or suspect your membranes may have ruptured, contact your maternity team.
Ultrasound can demonstrate reduced fluid, but clinical evaluation may be needed to determine whether the membranes have actually ruptured.
Why Does the Sonographer Check the Bladder?
Because fetal urine contributes significantly to amniotic fluid later in pregnancy.
The fetal bladder normally fills and empties.
If the bladder is visible and filling, that provides useful information about fetal urine production.
The sonographer may also reassess:
both kidneys
urinary tract
and
bladder appearance.
A single moment when the bladder happens to be empty does not automatically indicate a problem.
The overall examination matters.
Why Is Fetal Growth So Important?
A falling fluid measurement can become more significant when the fetus is also measuring small.
The combination of:
fetal growth restriction + low amniotic fluid
may raise concern about placental function.
The ultrasound may therefore assess:
- estimated fetal weight
- abdominal circumference
- interval growth
- amniotic fluid
And when indicated:
umbilical artery Doppler.
What Does Umbilical Artery Doppler Tell Us?
Umbilical artery Doppler evaluates resistance within the placental circulation.
If the fetus is small or placental dysfunction is suspected, Doppler provides information that a fluid measurement alone cannot.
So if fluid is falling, the question is not simply:
“What is today’s AFI?”
It may also be:
“How is the baby growing, and are the Dopplers reassuring?”
What If the Baby Is Growing Normally?
That is an important reassuring feature.
If:
growth is appropriate
fetal anatomy is reassuring
membranes appear intact clinically
and
fluid remains above the oligohydramnios threshold,
a lower AFI than the previous scan may not represent a serious problem.
Your provider may simply recommend follow-up depending on the gestational age and clinical context.
Does Amniotic Fluid Naturally Decrease Near the End of Pregnancy?
Fluid volume changes with gestational age.
It generally increases through much of pregnancy and later tends to decline as term approaches.
Therefore, an AFI that is lower at 38 weeks than it was earlier in pregnancy is not automatically abnormal.
The important issue is whether the measurement is appropriate for the clinical situation and whether it meets criteria for oligohydramnios.
Can the Fluid Go Back Up?
Yes.
A later ultrasound may show a higher measurement.
That can occur because of:
- true physiological variation
- maternal hydration
- fetal urine production
- fetal position
- different measurable pockets
- measurement variability
This is another reason one borderline measurement should be interpreted cautiously.
But if true oligohydramnios is present, follow-up should be guided by your healthcare team rather than assuming it will resolve.
Should I Drink More Water Before My Next Ultrasound?
Staying appropriately hydrated is reasonable.
But don’t try to manipulate the ultrasound result by rapidly drinking excessive amounts of water.
The goal is not to “pass” an AFI measurement.
The goal is to understand the actual condition of the pregnancy.
If your provider gives you specific hydration advice, follow that guidance.
What If My Fluid Keeps Decreasing?
A consistent downward trend deserves closer attention, particularly if the measurement approaches oligohydramnios.
Your healthcare provider may reassess:
- fetal growth
- fetal kidneys and bladder
- membrane status
- maternal blood pressure
- placental function
- Doppler findings
- fetal wellbeing
The significance depends heavily on gestational age.
A downward trend at 40 weeks creates a different management question from the same trend at 25 weeks.
Will I Need NST or BPP Monitoring?
Possibly.
If fluid becomes genuinely low or there are additional concerns, later pregnancy surveillance may include:
NST — Nonstress Test
and/or
BPP — Biophysical Profile.
A BPP includes an ultrasound assessment of amniotic fluid along with fetal activity.
The monitoring schedule should be individualized.
Does Falling AFI Mean I Need to Deliver?
Not necessarily.
A decrease from one normal value to another normal value does not automatically mean delivery is needed.
Delivery becomes a consideration based on the complete picture:
gestational age
actual degree of low fluid
fetal growth
Doppler
fetal surveillance
membrane status
and
maternal conditions.
Near term, confirmed oligohydramnios may influence delivery timing.
Earlier in pregnancy, management may be very different.
A Simple Example
Imagine two patients.
Patient A
AFI decreases from:
14 cm → 9 cm
Baby is growing normally.
No fluid leakage.
Other findings are reassuring.
Patient B
AFI decreases from:
9 cm → 4 cm
Baby’s abdominal circumference is also falling across percentiles.
Umbilical artery Doppler is abnormal.
Both patients have a decrease in AFI.
But clinically, these are completely different situations.
That’s why:
the trend needs context.
Questions to Ask Your Doctor
If your fluid measurement has decreased, useful questions include:
- What was my previous AFI or DVP?
- What is it today?
- Are we comparing the same measurement method?
- Is today’s fluid actually abnormal?
- Is the baby growing normally?
- Are the kidneys and bladder normal?
- Could my membranes be leaking?
- Do I need Doppler ultrasound?
- Is this expected for my gestational age?
- Should the fluid be checked again?
- Do I need NST or BPP monitoring?
- Does this affect delivery timing?
And perhaps the most useful question:
“Is the lower number itself concerning, or is the rest of the pregnancy reassuring?”
