During an anatomy scan, parents sometimes notice a perfectly dark circle somewhere inside the baby’s abdomen.
Then comes the question:
“What is that black spot?”
On ultrasound, fluid usually appears black.
So a round or oval black structure may simply be a normal fluid-filled organ—the stomach, bladder, gallbladder, or part of the urinary tract.
But occasionally, we find a separate:
fetal abdominal cyst.
When that happens, identifying a cyst is only the beginning.
The more important question is:
Where is it coming from?
“Cyst” Describes an Appearance, Not a Diagnosis
This distinction matters.
On ultrasound, a simple cyst often appears:
- anechoic or black
- round or oval
- thin-walled
- fluid-filled
- without internal blood flow
But several different fetal structures can produce a cystic appearance.
So when I see a black round structure in the abdomen, I don’t immediately name it.
First, I establish the anatomy around it.
Step One: Is It Actually a Normal Organ?
Before calling anything a cyst, I look for the structures that are supposed to contain fluid.
Stomach
Usually seen in the left upper abdomen.
Gallbladder
A smaller elongated fluid-filled structure beneath the liver.
Bladder
Located centrally in the lower fetal pelvis and changes size as it fills and empties.
Renal pelvis
Fluid can become visible when the collecting system is dilated.
This first step prevents a normal anatomical structure from being mistaken for an abnormal cyst.
Step Two: Where Is the Cyst?
Location is one of our best clues.
I ask:
Is it upper or lower abdomen?
Right, left, or midline?
Near the kidney?
Near the liver?
Close to the bladder?
Within the fetal pelvis?
Does it seem connected to the bowel?
A cyst in the upper right abdomen has a very different differential diagnosis from a cyst deep in the fetal pelvis.
This is why location often matters more than size at the first examination.
Step Three: Is There Blood Flow?
Color Doppler can be extremely helpful.
A true simple cyst usually has:
no internal vascular flow.
But blood vessels can also look black on grayscale ultrasound.
Color Doppler helps us distinguish a cystic structure from:
- umbilical vessels
- portal or hepatic vessels
- vascular abnormalities
This is especially important when a structure lies near the liver or umbilical vein.
What Can Cause a Fetal Abdominal Cyst?
The possibilities depend strongly on fetal sex, gestational age, location, and associated findings.
Potential origins include:
ovarian cysts
gastrointestinal duplication cysts
mesenteric or omental cysts
renal or urinary tract abnormalities
choledochal cysts
hepatic cysts
and other less common cystic lesions.
But these do not all look the same.
Ovarian Cysts Are an Important Cause in Female Fetuses
When a cyst is found in the lower abdomen or pelvis of a female fetus—especially later in pregnancy—one possibility is:
a fetal ovarian cyst.
These are influenced by maternal and placental hormones.
Many are simple cysts.
Some decrease or disappear after birth as hormone levels fall.
However, larger ovarian cysts can occasionally develop complications such as:
torsion or hemorrhage.
This can change their ultrasound appearance.
Can Ultrasound Prove a Cyst Is Ovarian?
Not always.
The fetal ovaries themselves are usually difficult to identify directly.
Instead, the diagnosis is often based on:
- fetal sex
- pelvic or lower abdominal location
- cyst appearance
- relationship to the bladder and kidneys
- absence of another clear organ of origin
So prenatal imaging may appropriately describe:
“suspected ovarian cyst”
rather than claiming absolute certainty.
What Is a Duplication Cyst?
A gastrointestinal duplication cyst is a congenital cystic abnormality associated with the digestive tract.
On ultrasound, some duplication cysts may show a characteristic layered wall appearance sometimes called the:
double-wall or muscular-rim sign.
But not every lesion shows a textbook appearance.
Its relationship to nearby bowel can be an important clue.
What About a Choledochal Cyst?
A choledochal cyst involves abnormal dilation of the biliary tract.
Prenatally, it may appear as a cystic structure in the:
right upper abdomen.
But several other structures can occur in this region.
The gallbladder, hepatic vessels, bowel, and other cystic lesions must be considered.
Its anatomical relationship to the biliary system becomes particularly important.
Could It Be a Kidney Problem?
Yes.
A cystic structure near a kidney may actually represent part of the urinary collecting system.
For example, significant dilation of the:
renal pelvis or ureter
can sometimes create a cyst-like appearance.
That’s why I don’t assess an abdominal cyst without checking:
- both kidneys
- renal pelvises
- ureters when visible
- fetal bladder
- amniotic fluid
The urinary system has to be viewed as a whole.
Can a Cyst Change During Pregnancy?
Absolutely.
And that change can be diagnostically useful.
A cyst may:
shrink
remain stable
enlarge
or
change from simple to complex.
For example, a previously simple ovarian cyst may develop internal echoes after hemorrhage or torsion.
So follow-up is not simply about asking:
“Is it still there?”
We also ask:
“Does it still look the same?”
What Does “Complex Cyst” Mean?
A simple cyst contains clear fluid and typically appears uniformly black.
A complex cyst may contain:
- internal echoes
- septations
- debris
- fluid-fluid levels
- thicker walls
- solid-appearing components
A complex appearance doesn’t automatically mean a tumor.
Blood, torsion, inflammation, or other contents can alter how a cyst looks.
Again:
appearance needs context.
Does the Size Matter?
Yes—but size alone isn’t enough.
A small cyst in a critical anatomical location may deserve attention.
A larger simple cyst with reassuring surrounding anatomy may have a different prognosis.
What matters includes:
size + location + appearance + growth + effect on nearby organs.
A single diameter cannot tell the whole story.
Can an Abdominal Cyst Compress Other Organs?
A sufficiently large lesion can.
Depending on its location, a large cyst may displace:
- bowel
- bladder
- kidneys
- stomach
- other abdominal structures
This is one reason serial ultrasound may be recommended for larger lesions.
We are not just monitoring the cyst.
We are monitoring what the cyst is doing to its neighbors.
Why Is Fetal Sex Important?
Because fetal ovarian cysts are an important consideration in female fetuses.
If the fetus is male, an ovarian origin is obviously excluded.
That immediately changes the differential diagnosis.
This is one of those situations where fetal sex can genuinely help interpret an ultrasound finding rather than simply satisfy curiosity.
What Else Does the Sonographer Check?
When I find a fetal abdominal cyst, I usually assess:
Stomach
Is it normal and separate from the cyst?
Gallbladder
Can I identify it separately?
Liver
Does the cyst arise near or within it?
Kidneys
Are both normal?
Bladder
Does it fill and empty normally?
Bowel
Is there dilation or an apparent relationship to the lesion?
Amniotic fluid
Is the amount appropriate?
Fetal growth
Is growth reassuring?
Color Doppler
Is there internal or surrounding vascular flow?
And, importantly:
Is the rest of the fetal anatomy normal?
Why Might I Need Another Ultrasound?
Because time often clarifies fetal abdominal cysts.
On follow-up we can determine whether the lesion:
disappears,
stays stable,
grows,
or
changes internally.
Its relationship to nearby organs may also become easier to define as the fetus grows.
A follow-up scan therefore isn’t necessarily a sign that something serious is expected.
Sometimes we simply need more information.
Would Fetal MRI Be Needed?
Not for every abdominal cyst.
Ultrasound is usually the primary imaging method.
However, when a cyst is large, anatomically complicated, or its organ of origin remains uncertain, fetal MRI may occasionally provide additional information.
The decision depends on the individual case.
Will My Baby Need Surgery?
Many fetal abdominal cysts do not require prenatal treatment.
Some resolve spontaneously.
Others may need imaging after birth.
And certain persistent lesions may eventually require pediatric surgical evaluation.
Whether surgery is needed depends on:
what the cyst actually is—not simply the fact that a cyst was seen prenatally.
Questions to Ask Your Doctor
If a fetal abdominal cyst is found, useful questions include:
- Where exactly is the cyst?
- Does it appear simple or complex?
- Is there blood flow inside it?
- Which organ does it seem to arise from?
- Are the kidneys and bladder normal?
- Is the bowel normal?
- Is the gallbladder seen separately?
- Has the cyst changed in size?
- Is it affecting nearby organs?
- Do I need another ultrasound?
- Will my baby need an ultrasound after birth?
- Is pediatric surgery consultation likely to be needed?
These questions are much more informative than simply asking:
“How big is the cyst?”
